Don’t worry if you missed Thursday’s registration night, this is not a problem, although we do need all players registered as soon as possible. Below is the process you now need to undertake to make sure that your child is registered for the fall season.

All documents need to be mailed to Attn of Ollie Hilliker, PO Box 345, Princeton NJ 08540

Registration:

1. I have attached all of the documents that need to be filled out
   1. US Youth Soccer Medical Release form (Needs to be notorized)
   2. NJ Youth Soccer Player Membership form (LEAVE LEAGUE INFORMATION, PASS #, TEAM # BLANK. CLUB # is 9210.) Player name must match birth certificate/passport. No nicknames.   Players U10 and above should use cursive for their signature.
   3. Princeton SA Code of Conduct form (Signed by player & parent/guardian)
   4. Mid NJ Sage form (Signed by player & parent/guardian)
   5. Also, 2 copies of 1 x 1 picture of player (headshot) in COLOUR on photo quality paper & a COPY of player’s birth certificate or passport are needed.

These forms all need printing out and mailing to me.

2. 2^nd installment or final payment is needed. Go to the PSA online registration page. Log in using email/password or personal info. This takes you to the payment screen. Pay in full or make your second payment. Choose continue, confirm and print. 2^nd installment payment due June 15, final payment due July 15.

Uniform:

All players need to purchase new uniforms. Our new uniform sponsor is ADIDAS and our uniform supplier is Ewing Sports. Uniform fees are in addition to program fees. Uniform payment is not due to PSA. Uniform orders and payment will be processed online with Ewing after orders have been placed. All players can be sized for Uniforms at Ewing Sports Shop. Their address is Ewing Sports Center, 1325 Route 206 Skillman, NJ 08558. Order forms are also attached to this email. You MUST keep a copy of the order form and also mail us a copy just for our records. You will receive an email at a later date with the process of how you order the uniforms online.

Travel Development Academy Players are required to order the following kit at $80.00:

1. Home and Away uniforms (2 jerseys, 1 short, 1 sock)
2. Training tee (1 gray tee-shirt)

Travel Team Players are required to order the following kit at $225.00:

1. Home and Away uniforms (2 jerseys, 2 shorts, 2 socks)
2. Warm-up Suit (1 top, 1 pant)
3. Training tee (1 gray tee-shirt)

All players: Player #’s are assigned per team. Team managers will follow up with players regarding player number requests. Specific numbers will only be granted when team duplicates can be avoided.

UNIFORMS ARE NON-EXCHANGABLE – IT IS STRONGLY RECOMMENDED THAT YOU TAKE YOUR CHILD TO TRY ON UNIFORM PIECES FOR PROPER SIZING.
